High School Sports

If your child will be participating in any high school sport (Varsity or Junior Varsity and LHSAA sanctioned) he/she must complete and have on file all of the paperwork listed below. You may download the three forms from the three links below.
1. Medical History Evaluation Form (LHSAA)
2. Athletic Participation/Parental Permission Form (LHSAA)
3. Substance Abuse/Misuse Form (LHSAA)
4. Copy of Birth Certificate
5. Athletic Transportation Waiver

Grade 6th-8th Sports

If your child will be participating in a grade 6th-8th sport, then he/she will must complete and have on file all of the paperwork listed below. You may download the forms from the links below.

1. Medical History Evaluation Form (LHSAA)
2. Permission/Travel/Aftercare Form
3. Note: Only boys basketball players are required to have a birth certificate on file.
