High School Sports

If your child will be participating in any high school sport (Varsity or Junior Varsity and LHSAA sanctioned) he/she must complete and have on file all of the paperwork listed below. You may download the three forms from the three links below.
1. Medical History Form (LHSAA)
2. Athletic Participation/Parental Permission Form (LHSAA)
3. Substance Abuse/Misuse Form (LHSAA)
4. Copy of Birth Certificate

Junior High School Sports

If your child will be participating in a junior high sport (6th, 7th, or 8th grades and not affiliated with the LHSAA), then he/she will must complete and have on file all of the paperwork listed below. You may download the two forms from the two links below. Any junior high student that plays on a high school team must compete the paperwork for High School Sports listed above.

3. Copy of Birth Certificate
